At REGO Park Counseling, we understand that health is shaped not only by clinical care, but also by a person’s environment, stability, and access to essential resources. Through New York State’s Social Care Network (SCN) program, we help Medicaid members address their health-related social needs.
All Medicaid members are screened for unmet needs such as food insecurity, housing instability, lack of transportation, utility needs, and safety concerns.
A Social Care Navigator works directly with members to identify unmet needs, understand eligibility, connect to resources, and provide follow-up support.
